new post this morning in response to a new article in the Annals of Internal Medicine. It reported (almost incredulously) that patient-reports of adverse events are much higher than hospital records indicate. Well, duh! Why do you think that is?

If you've been following the CT scan radiation overdose story out of Cedars-Sinai, then this latest development is probably not new to you. For those who haven't heard, Cedars-Sinai subjected some 260 CT scan patients to dangerous levels of radiation over a period of 18 months. Seems everyone in the CT scan department failed to notice the machine was set incorrectly.
This week at least 50 of those patients were notified of yet another problem.…

You remember last summer when Medicare made the big announcement that it would no longer pay hospitals to fix the damage done by certain kiinds of preventable medical errors? Well, a new study concludes that this highly publicized program has already failed. Chalk up another win for the healthcare industry blunt end, and another loss for patients.

Don't know if you've been following the Hurricane Katrina incident involving hospital staff members of Memorial Medical Center in New Orleans who gave lethal drug overdoses to some 18 patients. I posted a video interview today over on HealthCare PSI on the story and I thought you'd like to give it a look.

I read a story in the Baltimore Sun a few days ago that stuck with me; you know, it's the kind of story you just can't get out of your head.

Hey, I don't know how many of you follow hospital reports for death rates (sounds kinda creepy, I know) but
USAToday has the most comprehensive analysis available to the public for more than 4.600 hospitals. Their latest survey includes numbers for death by heart attack, heart failure and pneumonia, plus readmission rates for the same conditions.
